Objective: Double-bundle and selective-bundle anterior cruciate ligament (ACL) reconstructions are increasingly performed to better reproduce the double-bundle anatomy of the native ACL and to improve knee stability and surgical outcomes. This article illustrates how to optimize visualization of the ACL bundle anatomy and the appearances of graft components and postoperative complications.

Conclusion: It is important for the radiologist to be familiar with the appearance of double-bundle and selective-bundle ACL reconstructions and associated complications.

Methods: Twelve human cadaveric knee specimens were mounted with the femur on a custom-made fixation device allowing free range of motion in the knee joint for testing. Using a sensitive pressure film (Tekscan) patellofemoral contact pressure was measured in 15° intervals during a dynamic flexion movement from 0°-90° in the native state, in cut MPFL and after MPFL-reconstruction with a gracilis tendon.

Purpose: Reconstruction of the anterior cruciate ligament (ACL) is the most frequently performed reconstructive surgery in the knee. Biomechanical studies have shown that double bundle (DB) reconstruction is better than single bundle (SB) reconstruction with regard to rotational stability.
