A standard posterior approach was performed in 5 fresh frozen cadaveric limbs. An anterior incision was then used to measure the distance of the femoral neurovascular structures to the anterior capsule. Magnetic resonance imaging (MRI) of 11 hips was also analyzed to validate our cadaveric method. Cadaveric results demonstrated that before dislocation the femoral artery and vein were closest to the anterior capsule (mean distance, 21 mm), and the femoral nerve was the furthest away (mean distance, 25 mm). These results were not significantly different from the MRI findings. When the hip was dislocated, these distances significantly increased. This study suggests that the neurovascular structures are relatively well protected during an anterior capsulotomy performed during hip resurfacing. The procedure may be safer if the capsulotomy is performed with the hip dislocated and in a flexed position while cutting the anterosuperior aspect and in an extended position while cutting the anteroinferior aspect.

Although previous work has demonstrated that oral contraceptive pill (OCP) use does not affect resting muscle sympathetic nerve activity (MSNA), growing evidence indicates that it attenuates neurogenic vasoconstriction. Despite these advances, it remains unknown how OCP use affects the ability of MSNA to dynamically control vascular tone and arterial blood pressure (BP) on a beat-by-beat basis. Thus, we tested the hypothesis that, compared with naturally menstruating females (MC), those using OCPs will exhibit attenuated sympathetic vascular transduction at rest.

A 33-year-old male patient developed distal femur chronic osteomyelitis with massive bone loss after an open grade-3b fracture. Following several failed treatments to eradicate infection, a tibial turn-up procedure was performed to provide a stable and functional stump. To avoid neurovascular problems, the popliteal vessels and sciatic nerve were moved medially, and the flap was rotated externally to decrease the collapse.

A previous study reported an increase in carotid-femoral pulse wave velocity (cfPWV) during an upright posture compared to the supine position, partly due to sympathetic activation. However, given that cfPWV is influenced by the transmural pressure (TMP) of the artery, which is elevated in the abdominal aorta in the seated posture due to the increased hydrostatic pressure. Thus, it remains unclear whether this increased cfPWV reflects a true rise in arterial stiffness or is simply a result of the elevated TMP.
