A case of total pacemaker extrusion.

Europace

National Heart Hospital, Konyovitza 65, 1309 Sofia, Bulgaria.

Published: May 2009

Pacemaker extrusion is an extremely rare but serious complication. It is a consequence of an infection and/or small pacemaker pocket. We report a case of a total pacemaker extrusion due to a small pulse generator pocket.

Ipsilateral pneumothorax is a common complication of pacemaker implantation due to transvenous lead placement. We report a case of bilateral pneumothorax after unilateral subclavian vein cannulation for pacemaker implantation. An 85-year-old woman underwent dual-chamber pacemaker implantation for symptomatic atrioventricular block.

Unlabelled: Total extrusion of a pacemaker is rare and may result from skin and/or pocket infection or skin erosion because of fragility caused by actions such as scratching an itch. Total extrusion of a pacemaker may cause fatal arrhythmias, exacerbation of heart failure, and infection. We report the case of a 37-year-old man with a pacemaker implanted for complete atrioventricular block who presented with exacerbation of advanced heart failure due to complete extrusion of the pacemaker from the left groin and pacing failure.

Clinical Profile and Outcome of Patients with Cardiac Implantable Electronic Device-Related Infection.

Arq Bras Cardiol

June 2021

Universidade Federal de Minas Gerais - Departamento de Clínica Médica, Belo Horizonte, MG - Brasil.

Background: In recent years, the incidence of infections related to cardiac implantable electronic devices (CIED) has increased sharply, impacting mortality.

Objective: To verify the proportion of patients with CIED infection; to analyze their clinical profile and the variables related to the infection and its progression.

Methods: Retrospective and longitudinal observational study including 123 patients with CIED infection among 6406 procedures.

KCC2-Mediated Cl Extrusion Modulates Spontaneous Hippocampal Network Events in Perinatal Rats and Mice.

Cell Rep

January 2019

Faculty of Biological and Environmental Sciences, Molecular and Integrative Biosciences and Neuroscience Center (HiLIFE), University of Helsinki, 00014 Helsinki, Finland. Electronic address:

It is generally thought that hippocampal neurons of perinatal rats and mice lack transport-functional K-Cl cotransporter KCC2, and that Cl regulation is dominated by Cl uptake via the Na-K-2Cl cotransporter NKCC1. Here, we demonstrate a robust enhancement of spontaneous hippocampal network events (giant depolarizing potentials [GDPs]) by the KCC2 inhibitor VU0463271 in neonatal rats and late-gestation, wild-type mouse embryos, but not in their KCC2-null littermates. VU0463271 increased the depolarizing GABAergic synaptic drive onto neonatal CA3 pyramidal neurons, increasing their spiking probability and synchrony during the rising phase of a GDP.

The high energy demands of the substantia nigra pars compacta dopaminergic (DASNc) neurons render these neurons vulnerable to degeneration. These energy demands are a function of their long and extensively arborized axons and very large number of transmitter release sites, and are further augmented by their natural pacemaking activity. Pacemaking is driven by the rhythmic entry of Ca into the cell and, while the entry of Ca into the neuron stimulates energy (ATP) production, the extrusion of Ca conversely saps the energy that is generated.
