Highly active antiretroviral therapy (HAART) is imperative in managing human immunodeficiency virus (HIV) infections. HAART aims to inhibit viral replication and improve immunity. Antiretroviral therapy has led to significant improvement in CD4-T cell counts and reductions in viral load, leading to improved overall immune function, increased survival, and decreased frequency of opportunistic infections.

Acute lymphoblastic leukemia (ALL) poses a significant challenge due to its high relapse rate despite initial chemotherapy. Cell therapy plays an important and promising role in refractory ALL cases. The aim is to present a complex case of a 20-year-old male patient with relapsed ALL and to explore the different therapeutic options of cellular therapy - allogeneic hematopoietic stem cell transplantation, donor lymphocyte infusion, and chimeric antigen receptor T - detailing the collection, processing, and infusion, as well as the associated complications and management strategies.

Although utilization of deceased donors with history of IMD warrants caution, there may be circumstances under which such donors could be considered as suitable organ donor candidates. We present the first known report of liver transplantation from a deceased donor with cystinosis.

We present a rare case of pseudohyponatremia in a 20-year-old male patient with adult T-cell acute lymphoblastic leukemia (ATLL). The patient was admitted for a mediastinal mass with superior vena cava syndrome and was receiving pegaspargase therapy. The pseudohyponatremia was found to be secondary to hypertriglyceridemia associated with the pegaspargase treatment.

Surgical management may be required, but further investigation is needed to understand the pathogenesis and optimize management in atypical and chronic cases, such as this case with a history of corrected intestinal malrotation.
