This article is a definitive review of the enforcement problems inherent in a tobacco-free campus, identifying clearly the depth of the problems and the true challenges they pose. It describes the roles to be played by patients, staff, the CEO, department heads, and clinicians, as well as security officers. It stresses that for the enforcement of a tobacco-free initiative to be successful, the security department must have the cooperation and support of the entire hospital community.
