Erectile dysfunction (ED) is one of the most popular and disturbing sexual problems, which impairs patients' health and affects their quality of life. Sildenafil citrate, the first oral PDE-5 inhibitor in the world, has established its efficacy and safety credit in both doctors and ED patients. The authors reviewed the results of related studies in the last decade and made deeper insights into the use of sildenafil citrate in such aspects as the general drug tolerance and its influence on the cardiovascular system, potentiality of concomitant diseases with co-administration of other drugs, long-term safety, and ocular and genital safety.

Male sexual dysfunction, characterised by reduced libido, ejaculatory issues and erectile dysfunction, often results from oxidative stress and enzymatic imbalance, notably involving phosphodiesterase type 5 (PDE5) and nitric oxide synthase (NOS). This study explores the therapeutic potential of β-caryophyllene (β-CBP), a sesquiterpene with antioxidant and anti-inflammatory properties, in mitigating paroxetine-induced sexual dysfunction in rats. Male Wistar rats were divided into nine treatment groups: control, paroxetine (20 mg/kg/day), sildenafil (20 mg/kg/day), β-CBP (10 mg/kg/day), β-CBP (20 mg/kg/day), paroxetine with β-CBP (10 mg/kg), paroxetine with β-CBP (20 mg/kg), paroxetine with sildenafil and β-CBP with sildenafil.

The Fenton technique is known for its high efficiency and minimal environmental impact; however, its degradation reaction process requires a sustained acidic environment. We describe a woman in her late 20s with newly diagnosed systemic lupus erythematosus (SLE), who presented with fulminant pulmonary arterial hypertension (PAH) requiring inotropic and extracorporeal support. She was established on triple pulmonary vasodilator therapy with concurrent aggressive immunosuppression; however, treatment was complicated by infection and diffuse alveolar haemorrhage, necessitating delays in immunosuppression and withdrawal of epoprostenol. Despite this, with ongoing suppression of her SLE, her pulmonary haemodynamics improved, with normal pressures on right heart catheterisation several months later allowing stepdown to sildenafil monotherapy.
