The presence of endogenous antibodies in the serum of some patients has long been known to be a potential source of interference in immunoassays. We report falsely increased whole blood tacrolimus concentrations using the antibody conjugated magnetic immunoassay from Siemens HealthCare Diagnostics in a liver transplant recipient due to the presence of circulating endogenous antibodies (possibly heterophilic antibodies). Estimation of whole blood tacrolimus concentrations from the washed erythrocytes concentrations is proposed as a tentative approach for obtaining reliable results using the antibody conjugated magnetic immunoassay assay in these cases, leading to analogous blood tacrolimus concentrations to those produced by the microparticle enzyme immunoassay from Abbott Laboratories.

Background: Epstein-Barr virus (EBV) chronic high viral load (CHVL) may be defined by >16 000 copies/mL whole blood or >200 copies/10 peripheral blood mononuclear cells in >50% samples exceeding 6 mo. EBV CHVL has only been characterized in a few small pediatric studies, with heterogeneous results and unclear clinical significance.

Methods: This single-center observational study evaluated adult and pediatric kidney transplant recipients transplanted between 2010 and 2021 on tacrolimus/mycophenolate-based/prednisone immunosuppression.

Department of Oral and Maxillofacial-Head and Neck Oncology, Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine, Shanghai, China.

Background/purpose: Discoid lupus erythematosus (DLE) serves as an interdisciplinary disease involved in dermatology and stomatology in many cases. The purpose of this study was to analyze the scientometric characteristics and research trends of DLE.

Materials And Methods: All the papers on DLE were comprehensively retrieved from the Scopus database.

Introduction: Efgartigimod has been approved for the treatment of acetylcholine receptor antibodies-positive generalized myasthenia gravis (AChR-Ab+gMG), but its efficacy in patients with ocular myasthenia gravis (OMG) is not known.

Case Presentation: We describe 2 cases of patients with AChR-Ab+ OMG who showed unfavorable responses to corticosteroids and tacrolimus. Within 2 weeks of initiating efgartigimod, both patients showed rapid improvement and minimal symptom expression was achieved in weeks 3 to 4, which was maintained up to week 12.

Intra-patient variability in immunosuppressive blood drug concentrations is a potential biomarker in managing organ transplant patients. However, the association between the time in therapeutic range of tacrolimus blood concentrations and its efficacy in preventing graft-versus-host disease remains unknown. In this study, we analyzed the relationship between the time in therapeutic range of tacrolimus blood concentrations and its efficacy in acute graft-versus-host disease prophylaxis in patients undergoing allogeneic hematopoietic stem cell transplantation.

Although granulomatous interstitial nephritis (GIN) is a rare histological finding in kidney transplants, the joint occurrence of GIN and focal segmental glomerulosclerosis (FSGS) has not, to our knowledge, been reported in the literature. We report a case of GIN and de novo FSGS in kidney transplant recipients leading to allograft failure. A 69-year-old male with a history of end-stage renal disease (ESRD) of unknown etiology, as well as liver failure from hepatitis B and C co-infection, initially had a living unrelated kidney transplant (LURT) in 2007 and subsequently received both liver and kidney transplants (SLKTs) in 2017.
