Neglected posterior dislocation of the elbow.

Injury

Department of Orthopaedic Surgery, Faculty of Medicine, Zagazig University, Egypt.

Published: February 2009

The study included two adults and four children who underwent open reduction for subacute, untreated posterior dislocation of the elbow. Elbow stiffness was the main indication for surgery, and in all cases elbow movement was inadequate for the activities of daily living. Mean time from injury was 2 months and mean follow-up was 19 months. At final follow-up there were no cases of instability; according to the Mayo Elbow Performance Index(14), four outcomes were excellent, one good and one poor; the mean score improved from five preoperatively to 83 postoperatively, mean elbow flexion from 18 degrees to 93 degrees and mean pronation-supination from 20 degrees to 119 degrees. Complications included pin site infections in one case, treated by oral antibiotics and dressings, and severe degenerative changes with myositis ossificans in one case. Open reduction and V-Y muscleplasty followed by a supervised physical therapy programme can restore elbow function and stability in neglected elbow dislocations presenting up to 3 months after injury.

Posterior sternoclavicular joint (SCJ) dislocation is a rare but potentially life-threatening injury due to its proximity to critical mediastinal structures. Early diagnosis and prompt management are essential to prevent severe complications such as vascular or respiratory compromise. We report a case of a 23-year-old male who presented to our emergency department five days after a high-energy motor vehicle accident with isolated, closed posterior dislocation of the SCJ.

Background: Despite advancements in prosthetic designs and surgical techniques, patellar dislocation remains a rare but significant complication following total knee arthroplasty, with an incidence ranging between 0.15% and 0.5%.

Introduction And Importance: Neglected posterior hip dislocations in adults are rare, particularly when untreated for years. In developing nations, patients often rely on traditional bone setters, leading to delayed diagnosis and increased complications. Adult hip dislocations carry a higher risk of avascular necrosis and require complex treatments.

Article Synopsis
  • The study evaluated the long-term outcomes of scleral fixation of aniridic black diaphragm intraocular lenses (BDIOL) in 38 eyes affected by aniridia and aphakia due to trauma or congenital conditions.
  • After an average follow-up of 28 months, there was a significant improvement in visual acuity, and patients reported reduced glare and photophobia, though some experienced complications like elevated intraocular pressure and corneal decompensation.
  • The findings suggest that scleral fixation of BDIOL is an effective solution for patients lacking capsular support, providing good vision quality with a low rate of complications, regardless of prior surgical history.

Background: Anterior labroligamentous periosteal sleeve avulsion (ALPSA) lesions are associated with recurrent shoulder instability and higher rates of failure after capsulolabral repair compared with similarly treated Bankart lesions. Although these lesions can portend poor outcomes, there are limited data on the associated conditions and postoperative course in a young, active population.

Purpose: To evaluate the mid- to long-term clinical course and failure rates after ALPSA repairs and assess features associated with these outcomes.
