Mesenchymal hamartoma of the liver is the second most common benign liver tumor of childhood. Ultrasonography demonstrates a cystic lesion that may be very large. Computed tomographic scan will further establish the location and its relationship to vascular anatomy. Most will be amenable to gross total resection with clear margins. To illustrate the radiological and gross operative features, we present a 9-month-old male infant with a mesenchymal hamartoma with classic features.

Background: Liver transplantation for unresectable, benign hepatic lesions is rare. Hepatic mesenchymal hamartomas (HMH) are benign, cystic tumors that arise mostly in pediatric populations and can cause compressive symptoms. Hepatic mesenchymal hamartoma (HMH) is an uncommon, benign liver tumor predominantly affecting children under three years of age. It is characterized histologically by disorganized mesenchymal stroma, abnormal bile ducts, blood vessels, and hepatocytes. HMH can present as a large cystic mass, a solid mass, or a combination of both.
