Article Abstract

Clinical application of artificial oxygen carriers as a substitute for blood transfusion has long been expected to solve some of the problems associated with blood transfusion. Use for oxygen delivery treatment for ischemic disease by oxygen delivery has also been examined. These prospective applications of artificial oxygen carriers are, however, still in development. We have developed liposome-encapsulated hemoglobin (LEH), developmental code TRM-645, using technologies for encapsulation of concentrated hemoglobin (Hb) with high encapsulation efficiency as well as surface modification to achieve stability in circulating blood and a long shelf life. We have confirmed the basic efficacy and safety of TRM-645 as a red blood cell substitute in studies on the efficacy of oxygen delivery in vivo, and the safety of TRM-645 has been studied in some animal species. We are now examining various issues related to clinical studies, including further preclinical studies, management of manufacturing and the quality assurance for the Hb solution and liposome preparations manufactured by the GMP facility.

Continuous microenvironment modulation is an ongoing challenge in wound dressing, which includes excessive exudate absorption, oxygen delivery, bacterial inhibition and angiogenesis. Herein, we developed an construction strategy to fabricate a self-retaining double-layered wound dressing, where the top layer precursor was composed of Ca-containing polyvinyl butyral (PVB) solution dispersed with hydroxypropyl methylcellulose (HPMC) particles, and the bottom one consisted of sodium alginate (Alg) solution blended with Ag-doped mesoporous bioactive glass powders (Ag-MBG). When in use, both precursors were simultaneously squeezed out from the twin nozzles connected to the individual chambers of a twin-chambered syringe, whereby Ca in the top layer rapidly migrated downwards to crosslink Alg in the bottom layer, leading to the formation of an Alg/Ag-MBG (AA) functional hydrogel for filling an irregular wound.

Islet transplantation and more recently stem cell-derived islets were shown to successfully re-establish glycemic control in people with type 1 diabetes under immunosuppression. These results were achieved through intraportal infusion which leads to early graft losses and limits the capacity to contain and retrieve implanted cells in case of adverse events. Extra-hepatic sites and encapsulation devices have been developed to address these challenges and potentially create an immunoprotective or immune-privileged environment.
