Echocardiography
November 2018
2nd Department of Medicine - Department of Cardiovascular Medicine, First Faculty of Medicine, Charles University in Prague and General University Hospital in Prague, Prague, Czech Republic.
Aim Of The Study: To assess the diagnostic utility of a simplified approach to relative apical sparing of longitudinal strain (RAS LS) using only an apical four-chamber view (A4C) in patients with AL amyloid cardiomyopathy (ALAC).
Methods: We retrospectively evaluated echocardiographic recordings of 20 patients with ALAC, 20 patients with Fabry disease-related cardiomyopathy (FD), and 20 patients with concentric hypertensive left ventricular hypertrophy (HLVH) matched for mean LV mean thickness. Peak segmental LS values of the interventricular septum and lateral LV wall were measured in the A4C using two-dimensional speckle-tracking echocardiography.

Background: Pseudoinfarction is one of the most common electrocardiographic characteristics in cardiac light-chain (AL) amyloidosis. The aim of the present study was to analyze the prognostic significance of pseudoinfarction and define the relation between pseudoinfarction and clinical parameters in cardiac AL amyloidosis.
Methods: A total 110 consecutive patients who presented with a diagnosis of cardiac AL amyloidosis and without a positive history of coronary disease between 2010 and 2014 were enrolled.

Objectives: This study analyzed the utility of electrocardiographic (ECG) and echocardiographic findings in the diagnosis of amyloidosis proven by endomyocardial biopsy.
Background: Cardiac amyloidosis is associated with characteristic ECG and echocardiographic changes, yet each finding alone is relatively nonspecific. A combination of noninvasive prognostic parameters would be desirable for this tissue-based diagnosis.

A 64-year-old male who received an implantable cardioverter defibrillator with epicardial patch electrodes in 1990 was admitted to the emergency room because of recurrent defibrillator shocks. During the following diagnostic work-up a remarkable discrepancy between coronary angiography (no significant stenosis), 201Thallium-SPECT imaging (antero-septal persistent defect) and PET imaging (no 18FDG defect) was noticed. In order to assess the impact of the epicardial defibrillator patch electrodes on the imaging, we performed SPECT and PET measurements using a thorax phantom.
