The bird-face deformity of a severe class II, high Frankfort-mandibular plane angle with significant retrogenia, often associated with diminutive condyles and reduced posterior face height, poses many challenges to the orthognathic surgeon. Of greatest concern in these patients is the degree of mandibular advancement required and the potential for relapse. The sagittal split osteotomy is the workhorse of mandibular surgery but does not allow significant lengthening of the ramus, which is desirable in this group of patients.

Objectives: To compare the feasibility and efficacy of two methods of local anaesthesia with procaine 2% for flank laparotomy in cattle. MATERIAL UND METHODS: A total of 100 cattle undergoing laparotomy were divided into two groups of 50: one group underwent a modified infiltration anaesthesia (MIA) technique consisting of an incisional line block combined with an inverted L-block, and the other group underwent proximal paravertebral anaesthesia (PPVA). Indications for laparotomy were displaced abomasum, caecal disorders, rumenotomy and exploratory laparotomy.

Background/aims: The purpose of the present study was to clarify the indications for a thoracoabdominal right oblique approach and an inverted L incision in posterior segmentectomy of the liver for hepatocellular carcinoma.
Methodology: Forty-six patients with hepatocellular carcinoma who underwent posterior segmentectomy with thoracotomy were divided into 2 groups according to the incision: thoracoabdominal right oblique incision (group A, n = 17) and inverted L incision (group B, n = 29). The perioperative factors of the patients in the 2 groups were compared retrospectively.

The chief benefits of small skin incisions are reduced patient discomfort, accelerated recovery, and cosmetic satisfaction without compromising the quality of surgery. Since April 1997, the lower ministernotomy approach without femoral cannulation has been performed in 43 patients in the authors' institutions. The indications for this approach were initial single valve surgery and secundum-type atrial septal defect.
