Background Context: Avascular necrosis is a commonly described condition caused by a disruption of blood supply to the bones, resulting in necrosis. Although common in joints of the extremities, it is seen less often in the spine. Risk factors for avascular necrosis include steroid use, alcohol consumption, smoking, scuba diving, thrombosis, hypercoagulability, and hypertension.

Purpose: The purpose was to report an unprecedented case of avascular necrosis of the lumbar facet joints and bilateral facet fractures.

Study Design: This is a case report.

Methods: The patient underwent L3-S1 decompression and L5-S1 discectomy, during which time avascular necrosis of the superior articular process of the L3-L4 facet joints was discovered. The patient then underwent spinal fusion with pedicle screw instrumentation. Pathologic examination of both right and left facet joints confirmed the diagnosis of avascular necrosis.

Results: At 19-month follow up, the patient's leg and back pain had significantly improved. His spine appeared fused with no instability or implant failure.

Conclusion: We have presented a case of avascular necrosis of L3-L4 facet joints resulting in fracture and instability at the L3-L4 level of the spine, which was stabilized with an L3-L4 pedicle screw spinal fusion.

Recent Insights Into Wnt-Related tRNA-Derived Fragments (tRFs) in Human Diseases.

J Cell Biochem

January 2025

Key Laboratory of Novel Targets and Drug Study for Neural Repair of Zhejiang Province, School of Medicine, Hangzhou City University, Hangzhou, Zhejiang, China.

tRNA-derived fragments (tRFs) are a newly recognized class of small noncoding RNAs (sncRNAs) that play significant roles in various diseases. The Wnt pathway plays a key role in various physiological processes such as embryonic development, tissue renewal and regeneration. In the regulation of Wnt/β-catenin, Forkhead box k1(FOXK1), Frizzled class receptor 3 (FZD3), and Wnt5b can be targeted and inhibited by three tRFs: tRF3008A targets FOXK1 to inhibit colorectal cancer (CRC), 5'-tiRNAVal targets FZD3 to inhibit breast cancer (BrC), and tRF-22-8BWS7K092 targets Wnt5b to induce ferroptosis in lung cells.

Introduction This study evaluated hip joint dynamic instability in patients with non-traumatic osteonecrosis of the femoral head (ONFH) with extensive lesions, who had undergone anterior rotational osteotomy (ARO) and high-degree posterior rotational osteotomy (HDPRO), based on the femoral head translation observed by computed tomography (CT) at 0° and 45° hip flexion. Materials and methods Medical records of patients who had undergone transtrochanteric rotational osteotomy for non-traumatic ONFH were retrospectively reviewed to identify patients who had undergone CT examinations six weeks post-operatively. In all, 64 hips (60 patients; 19 men and 41 women), comprising 36 hips treated with HDPRO and 28 hips treated with ARO, respectively, were included.

Background: Osteopenia (ON) and osteoporosis (OP) are highly prevalent among postmenopausal women and poses a challenge for early diagnosis. Therefore, identifying reliable biomarkers for early prediction using metabolomics is critically important.

Methods: Initially, non-targeted metabolomics was employed to identify differential metabolites in plasma samples from cohort 1, which included healthy controls (HC, n = 23), osteonecrosis (ON, n = 36), and osteoporosis (OP, n = 37).

Background: Pediatric femoral neck fractures (PFNF) are rare but associated with a high rate of serious complications such as avascular femoral head necrosis (AVN). Major risk factors and prognostic tools for an AVN are still unclear. As AVN is a devastating complication, this study aims to evaluate the predictors for AVN following a PFNF.

Investigating the preventive effects of pulsed electromagnetic fields on glucocorticoid-induced osteoporosis in rats.

Sci Rep

January 2025

Department of Spine and Spinal Cord Surgery, Henan Provincial People's Hospital, People's Hospital of Zhengzhou University, People's Hospital of Henan University, No. 7 Weiwu Road, Jinshui District, Zhengzhou, 450003, Henan, China.

The use of pulsed electromagnetic field (PEMF) has demonstrated effectiveness in the management of femoral head osteonecrosis as well as nonunion fractures; however, the effects of PEMF on preventing glucocorticoid-induced osteoporosis (GIOP) have not been extensively studied. The aim of this investigation was to explore the effectiveness of PEMF stimulation in averting GIOP in rats and uncover the potential fundamental mechanisms involved. A total of seventy-two adult male Wistar rats composed the experimental group and were subsequently assigned to three groups for treatment.
