This report describes the removal of a large, symptomatic leiyomyosarcoma arising from the proximal femoral vein necessitating removal of the femoral venous and arterial circulations. Reconstruction was accomplished with autologous vein and initial coverage with ipsilateral rectus abdominus flap. Persistent, early postoperative lymphatic leak and groin sepsis secondary to staph aureus was managed with reoperation and coverage with contralateral rectus abdominus muscle flap. Negative pressure dressing device was used as a wound management adjunct and splint thickness skin graft providing final successful coverage. Two years following the operation the patient was without evidence of disease, had a patent vascular reconstruction and a well healed groin. Femoral vein leiyomyosarcoma is a rare vascular tumor, which is especially challenging to manage in the proximal location. Successful outcome is predicated on revascularization with autologous vein and on a multidisciplinary approach using various soft tissue coverage strategies and wound management adjuncts.

Simultaneous pancreas-kidney (SPK) transplantation is a recognized treatment for patients with insulin-dependent diabetes and advanced chronic kidney disease or end-stage renal disease (ESRD), offering significant survival benefits. However, it is associated with a higher risk of venous thrombosis, which can jeopardize the survival of the pancreaticoduodenal graft. This case report describes a patient with type 2 diabetes, hypertension, and ESRD who developed acute, occlusive deep vein thrombosis (DVT) involving the right common femoral, profunda femoral, and greater saphenous veins on postoperative day 1 (POD1) following a deceased donor SPK transplant, despite systemic prophylactic anticoagulation.

Transcarotid artery revascularization (TCAR) is a novel method to treat severe stenosis of the carotid artery with minimal embolization. During TCAR, flow reversal system redirects blood from the internal, external, and common carotid arteries into the femoral vein through a filter system to prevent debris and microparticles from entering the cerebral circulation. Transcranial Doppler (TCD) monitoring allows real-time detection of blood flow in the cerebral arteries during the operation and informs the surgeon of flow changes or possible emboli.

Background/objective: Obesity and rapid weight loss are risk factors for developing deep vein thromboses (DVTs). Our aims were to present a patient who developed extensive DVT after relatively rapid and severe weight loss that followed taking tirzepatide and to raise the awareness among health care professionals regarding the risk of DVT that can be associated with significant weight loss due to these agents.
Case Report: We present the case of a 20-year-old young man, with raised body mass index of >35 kg/m, who was initiated on tirzepatide treatment for weight loss, with 12-kg weight lost over 6 weeks.
