Background: Vulvar Paget's disease is a relatively rare gynecologic neoplasm that is problematic because of its propensity to recur.
Case: An 80-year-old woman was found to have recurrent Paget's disease of the vulva. She was initially diagnosed 12 years previously and had had multiple resections for the disease. She was found to have histologically proven Paget's disease and underwent a course of topical immune modulator therapy with imiquimod. Biopsy proved histologic regression of the disease progress. The patient remained without recurrence 12 months after therapy.
Conclusion: In select patients imiquimod may be used to treat or facilitate treatment of vulvar Paget's disease.
