[Molecular biology of heart failure].

Heart failure is a complex disorder involving maladaptive responses that result in defective regulation and function of multiple biological systems. Adequate understanding of these processes is basic for the development of novel therapeutic approaches. This review, directed to the molecular biology of the heart, is divided in three sections, with some redundancy between them: hypertrophy and remodeling, molecular composition of the failing heart, and molecular mechanisms leading to heart failure.

Adeno-associated viral (AAV) vectors are increasingly used for preclinical and clinical cardiac gene therapy approaches. However, gene transfer to cardiomyocytes poses a challenge due to differences between AAV serotypes in terms of expression efficiency and . For example, AAV9 vectors work well in rodent heart muscle cells but not in cultivated neonatal rat ventricular cardiomyocytes (NRVCMs), necessitating the use of AAV6 vectors for studies.

Focused ultrasound presents a non-viral, image-guided technique in which circulating intravascular microbubble contrast agents can reversibly enhance vascular permeability and gene penetration.
