Protein contact dermatitis is an allergic skin reaction induced principally by proteins of either animal or plant origin. The clinical presentation is that of a chronic dermatitis, and it is often difficult to differentiate between allergic contact dermatitis and other eczematous dermatoses. One distinguishing clinical feature is that acute flares of pruritus, urticaria, edema, or vesiculation are noted minutes after contact with the causative substances. Additionally, the patch-test result is typically negative, and the scratch- or prick-test result is positive. The pathogenesis of protein contact dermatitis is unclear but may involve a type I (immunoglobulin E [IgE], immediate) hypersensitivity reaction, type IV (cell-mediated delayed) hypersensitivity reaction, and/or a delayed reaction due to IgE-bearing Langerhans' cells. Management involves avoidance of the allergen.

Toluene-2,5-diamine sulfate (PTDS), also known as toluene diamine sulfate or 2,5-diaminotoluene sulfate, is an aromatic amine that can be used as a paraphenylenediamine (PPD) hair dye alternative. Patients may develop contact allergy to PTDS, and it is often omitted from standard patch test screening series, possibly leading to underdiagnosis. The American Contact Dermatitis Society named PTDS the Allergen of the Year for 2025, aiming to raise awareness of its dual role as both an allergen and an alternative for some PPD-allergic individuals.

The toxicokinetics of nitrosamines remain a mystery to this day, though it appears that the role of nitrosamines in potentiating the generation of mutations required for the onset of skin cancer continues to be a significant concern. Nitrosamines are mutagens, genotoxic substances, and mediators of phototoxicity/carcinogenicity, whose long-term daily usage, in the context of polypharmacy, can result in the parallel appearance of heterogeneous forms of skin cancer: keratinocytic and melanocytic. But a number of clinical observations suggest that it is the nitrosamines that potentiate the multiple occurrences of skin cancer over the years, or recurrences of skin cancer localized in areas exposed to solar radiation.

We report a case of an industrial homeworker diagnosed with allergic contact dermatitis by UV-curing acrylic resin for crafts. Approximately 2 months after a female in her 40s started producing handicrafts using resin, itchy desquamative erythema and vesicles occurred on her eyelids and palms. The course of the symptoms suggested that her dermatitis was occupational origin.
