Purpose: To present a technique that addresses the problem of passing guiding catheters through a stent-graft fenestration and into the target orifice over the wire within the visceral vessel when the fenestration is remote from the top cap.

Technique: A compliant molding balloon is introduced into the main body of the stent-graft and inflated above the fenestration to fill the redundant space in the proximal stent-graft to prevent upward angulation and subsequent displacement of the wire from the target vessel when a less flexible catheter is passed.

Conclusion: This technique is useful in cases with tortuous and complex anatomy and may reduce overall procedure times.

The biomechanics of turgor pressure.

If you ever forget to water your houseplant, you may find its leaves getting soft and droopy - if you water it again in time, the leaves may stiffen, spring back up, and resist gravity. During this recovery, plant cells absorb water and build up an intracellular pressure, called turgor pressure, similar to inflating a balloon. Turgor pressure is an intrinsic component of plant physiology, and its biomechanical role as the 'hydroskeleton' is generally appreciated either statically in structural stability, like leaves resisting gravity, or dynamically in rapid motions, like Venus flytrap snapping, Mimosa closing, or stomatal opening.

Purpose: Ultrasound-guided puncture and punch biopsy pose a particular challenge in ultrasound examination training. These techniques should be learned and performed several times using a simulation model that is as realistic as possible before being applied to patients. While the use of agar-agar-based models is extensively documented in the literature, there is a discernible gap in publications specifically addressing their use in punch biopsy and puncture.

Purpose: To evaluate the efficacy and safety of selective aneurysmal sac neck-targeted embolization in endovascular aneurysm repair (EVAR) in patients with a hostile neck anatomy (HNA).

Materials And Methods: Between October 2020 and June 2022, patients with an abdominal aortic aneurysm (AAA) and HNA who underwent EVAR with a low-profile stent graft and a selective aneurysmal sac neck-targeted embolization technique were analysed. An HNA was defined by the presence of any of the following parameters: infrarenal neck angulation > 60°; neck length < 15 mm; conical neck; circumferential calcification ≥ 50%; or thrombus ≥ 50%.

Article Synopsis
  • The STABILISE technique is used for treating type B aortic dissections and has shown good results, but carries risks of severe complications.
  • A specific case was reported where a patient experienced an infrarenal aortic rupture during the procedure due to balloon molding.
  • Healthcare providers are advised to exercise caution and use a smaller semi-compliant balloon to reduce the risk of such ruptures when applying the STABILISE technique.

Background: Abdominal Aortic Aneurysm (AAA) is a balloon-like dilatation that can be life-threatening if not treated. Fabricating patient-specific AAA models can be beneficial for investigations of hemodynamics, as well as for pre-surgical planning and training, testing the effectiveness of different interventions, or developing new surgical procedures. The current direct additive manufacturing techniques cannot simultaneously ensure the flexibility and transparency of models required by some applications.
