Urinary bladder transplantation in humans - current status and future perspectives.

Nat Rev Urol

September 2024

Department and Chair of Urology and Andrology, Ludwik Rydygier Collegium Medicum, Nicolaus Copernicus University, Bydgoszcz, Poland.

Urinary bladder vascularized allograft transplantation in humans is currently extensively being investigated worldwide, owing to the theoretical potential of this approach as a therapeutic option for individuals with end-stage, non-oncological bladder conditions or congenital bladder pathologies. To date, a successful attempt at urinary bladder autotransplantation was carried out in a heart-beating brain-dead research human donor. The robot-assisted surgical technique was shown to be optimal for performing this procedure, achieving a good performance in terms of both bladder allograft collection as well as vascular, ureterovesical and vesicourethral anastomoses.

Background And Objective: The artificial urinary sphincter (AUS) remains the gold standard for treatment of stress urinary incontinence (SUI). However, highly complex patients such as those with bulbar urethral compromise, bladder pathology, and lower urinary complications pose a particular challenge for the surgeon. In this article, we will address critical risk factors and synthesize existent data across relevant disease states to support surgeons in successful management of SUI in high-risk patients.

Article Synopsis
  • Robotic-assisted laparoscopic prostatectomy (RALP) requires careful fluid management during surgery to ensure a clear view and prevent complications like airway edema.
  • The study tested a specific fluid regimen that maintains low infusion rates during critical surgery phases but allows for increased fluids afterward, aiming to see if this would affect kidney function post-surgery.
  • Results showed no significant increase in serum creatinine levels post-surgery, with most patients having a good view during surgery and minimal cases of acute kidney injury, indicating that the fluid regimen was effective and safe.
