The hot quadrate sign is defined as an intense arterial enhancement in the hepatic quadrate lobe, most frequently encountered on CT angiograms in patients with central venous occlusion. It has also been described as focal uptake on technetium-99m (Tc99m) sulfur colloid scans. We present an unusual case of focal uptake in the hepatic quadrate lobe on a ventilation-perfusion (V/Q) scan, corresponding to the hot quadrate sign in a 42-year-old patient with chronic kidney disease and central venous occlusion.

A 67-year-old woman presented with shortness of breath and a ventilation/perfusion scan was performed. Initial images demonstrated mismatched bilateral apical defects that would be classified as high probability for pulmonary emboli. However, it was unusual that the defects were only in the bilateral apices.

Pulmonary artery (PA) vasoconstriction resulting from pulmonary ventilation/perfusion imbalance is infrequently detected with computed tomography (CT). An 85-year-old woman presented to the emergency room with dyspnea and desaturation, despite oxygen supply. A CT pulmonary angiography (CTPA) revealed massive central bronchial mucoid impaction in all but the right upper bronchus.

A 79-year-old man with prior bioprosthetic mitral valve replacement presented with progressive shortness of breath and was found to have right upper pulmonary vein stenosis and paravalvular leak diagnosed with the use of multimodal imaging. The patient underwent balloon angioplasty, stenting of the pulmonary vein, and paravalvular leak closure with ultimate resolution of symptoms. ().

A 48-year-old woman was referred for a bone scan as an assessment of bone metastasis from breast cancer. Surprisingly, two hot spots of lung uptake were present in the left lung without any abnormality on CT slices. No history of pulmonary disease was observed.
