Epstein-Barr virus (EBV) infection is a benign disease, which may occasionally be fatal, particularly in children and immunocompromised patients. Epstein-Barr virus infection is rare in elderly subjects and seems to have a self-limited course. A case of fatal liver failure due to chronic active EBV infection in a 75-year-old man is described. The etiology was established postmortem by cellular expression of EBV-DNA in liver and lymphatic tissue. This patient meets the diagnostic criteria of a case definition for a highly probable "severe chronic active EBV syndrome." Either suspected or unsuspected, the long-term prognosis of this syndrome is poor and its mortality high.

Chemotherapy-induced peripheral neuropathy (CIPN) is a serious side effect of anticancer agents with limited effective preventive or therapeutic interventions. Although fenofibrate, a peroxisome proliferator-activated receptor-alpha (PPARα) agonist, has demonstrated neuroprotective and analgesic properties, its clinical utility is hindered by low receptor affinity, poor subtype selectivity, and suboptimal bioavailability. A190, a highly selective and potent nonfibrate PPARα agonist, offers a promising alternative but is limited by poor aqueous solubility, resulting in reduced oral bioavailability and therapeutic efficacy.

AMP-activated protein kinase (AMPK) is a central mediator of cellular metabolism and is activated in direct response to low ATP levels. Activated AMPK inhibits anabolic pathways and promotes catabolic activities that generate ATP through the phosphorylation of multiple target substrates. AMPK is a therapeutic target for activation in several chronic metabolic diseases, and there is increasing interest in targeting AMPK activity in cancer where it can act as a tumor suppressor or conversely it can support cancer cell survival.
