Objective: To report a case of a 46,XX male with an intratubular undifferentiated germ cell neoplasia within an extra-abdominal gonad.

Design: Case report.

Setting: Molecular, cytogenetic, pathologic, and clinical units of three tertiary hospitals.

Patient(s): A male with ambiguous genitalia at birth and descended testes observed in a pediatric endocrinology setting.

Intervention(s): Physical examination, hormonal assays, cytogenetic investigation, molecular analysis, surgical intervention for biopsies and bilateral orchiectomy, and pathologic evaluation.

Main Outcome Measure(s): Pathologic evaluation with immunostaining for placental alkaline phosphatase and C-kit.

Result(s): Conventional chromosome analysis revealed a 46,XXq- karyotype, and fluorescence in situ hybridization experiments with the SRY probe found a signal at the short arm of the deleted X chromosome. Molecular analysis indicated the presence of a portion of the short arm of the Y chromosome including the proto-oncogene TSPY. Pathologic evaluation of the gonads revealed an intratubular undifferentiated germ cell neoplasia.

Conclusion(s): This is the first case of a 46,XX male with descended testes in whom an intratubular undifferentiated germ cell neoplasia developed. When proposals of management in this subgroup of disorders of sexual differentiation are formulated, the risk of germ cell malignancy must be taken into account.

As an extraordinarily diverse group of bacteria, lactobacilli are now classified into several genera, many of which still include "Lactobacillus" in their names. Despite their names, this group of lactic acid bacteria comprises microorganisms that are crucial for human health, especially during the early development of the human microbiota and immune system. The interactions between lactobacilli and components of the mucosal immunity lead to its shaping and development, which is possibly considered a prime mover in the advancement of the human immune system.

Human papillomaviruses (HPVs) are responsible for the majority of sexually transmitted infections (STIs), some of which are oncogenic and can cause oropharyngeal or genital cancers. The HPV prevalence at the genital level varies according to the population studied but is higher in the seminal fluid of men suffering from idiopathic infertility than in the general population. The involvement of HPV in male infertility is supported by several studies suggesting that this virus can affect sperm quality by altering sperm DNA integrity, motility, number, viability, and morphology, and by inducing the production of anti-sperm antibodies (ASAs).

The pluripotent stem cell (PSC)-derived human primordial germ cell-like cells (PGCLCs) are a cell culture-derived surrogate model of embryonic primordial germ cells. Upon differentiation of PSCs to PGCLCs, multiple loci of HML-2, the hominoid-specific human endogenous retrovirus (HERV), are strongly activated, which is necessary for PSC differentiation to PGCLCs. In PSCs, strongly activated loci of HERV-H family HERVs create chromatin contacts, which are required for the pluripotency.
