Article Synopsis
  • - Chylopericardium is a rare condition resulting from lymphatic disruption, often arising from trauma or surgeries like coronary artery bypass grafting (CABG); it leads to fluid accumulation in the pericardial cavity.
  • - A 62-year-old man developed severe symptoms one week post-CABG, revealing chylous fluid in the pericardial space, which resulted in cardiac tamponade and ultimately led to his death despite aggressive medical interventions.
  • - The case highlights the rarity and complexity of diagnosing post-operative chylopericardium, emphasizing the necessity for prompt recognition and treatment strategies to manage such critical complications.

J Vasc Surg Cases Innov Tech

September 2021

Department of Plastic and Reconstructive Surgery, Graduate School of Medicine, Mie University, Tsu, Japan.

Chylothorax is a potentially fatal postoperative complication of neck, thoracic, and abdominal surgery in children. We report the case of a 3-month-old infant who developed persistent chylothorax with respiratory insufficiency successfully managed using a microsurgical technique and intraoperative embolotherapy. This was achieved using a combination of intermittent digital X rays and live near-infrared fluorescence imaging we have termed "dual imaging lymphangiography" to guide therapy in real time.
