Introduction: Sodium nitrite is a potent oxidizer, which may precipitate rapidly lethal methemoglobinemia. Prompt diagnosis and treatment may salvage otherwise fatal cases. It is unclear if emergency departments are prepared for increasing cases.

Background: Naphthalene is an aromatic hydrocarbon that potentially produces methemoglobinaemia but rarely causes hemolysis, especially in children with underlying glucose-6-phosphate dehydrogenase deficiency. Although ingestion of a single moth ball by an older child may not be life threatening, it can be fatal if ingested by a toddler.
Case Presentation: A 2-year-old Singhalese boy developed acute severe hemolysis and methemoglobinaemia following ingestion of a mothball.

Tumor lysis syndrome (TLS) is a critical oncologic emergency characterized by metabolic disturbances resulting from rapid cancer cell lysis. Rasburicase, a recombinant urate oxidase, is the primary treatment for hyperuricemia in TLS but poses a risk for methemoglobinemia in patients with glucose 6-phosphate dehydrogenase (G6PD) deficiency. We present the case of a 59-year-old male with diffuse large B-cell lymphoma (DLBCL) who developed spontaneous TLS.
