The difficulty of anesthetic management of the critically ill, especially cardiovascular risk patients is discussed. The usually applied practice of general anesthesia or neuroaxial blockades for bilateral orchiectomy has a considerable cardiovascular risk profile. This case report presents an alternative procedure with an ilioinguinal nerve blockade completed by periscrotal infiltration. This is a generally unknown and uncommon minimally invasive locoregional procedure, which is generally judged to be safe, efficient, economical with low grade side effects or complications and suitable for outpatient surgery. Therefore it is an additional element to anesthetic management for nearly all operative scrotal procedures concerning cardiovascular risk patients.
