Background: Point-of-care ultrasound (POCUS) is a valuable skill for generalist physicians, nurse practitioners (NPs), and nurses; however, its utilization remains limited. This study was performed to investigate the current status, barriers, and facilitators of POCUS implementation among physicians, NPs, and nurses in family and hospital medicine in Japan and to identify differences in influencing factors between physicians and NPs/nurses.
Results: A web-based survey was distributed via the mailing lists of four major academic societies in general medicine in Japan-the Japanese Society of Hospital General Medicine, the Japan Primary Care Association, the Japanese Association for Home Care Medicine, and the Japan Society of Nurse Practitioner-from April to June 2024.

The SARS-CoV-2 virus infection, the most severe pandemic in recent human history, found healthcare systems around the world more or less unprepared. Adjusting to this challenge involved changes in the daily routines of healthcare systems, as well as the patients, once again highlighting the importance of primary care (family physician or general practitioner). In the context of the COVID-19 pandemic, the family doctor in Romania played a crucial role in patient management, rapidly adapting to the changes and challenges imposed by the state of emergency.

Thrombolysis in Myocardial Infarction (TIMI) Study Group, Cardiovascular Division Brigham and Women's Hospital and Harvard Medical School Boston MA USA.
Background: Epistaxis is common with antithrombotic therapy and is often troublesome to patients, yet its frequency, severity, and outcomes are poorly characterized.
Methods And Results: Effective Anticoagulation with Factor Xa Next Generation in Atrial Fibrillation-Thrombolysis in Myocardial Infarction 48 (ENGAGE AF-TIMI 48) randomized 21 105 patients with atrial fibrillation and CHADS2 risk score ≥2 to higher-dose edoxaban regimen (60 mg daily, dose-reduced to 30 mg), lower-dose edoxaban regimen (30 mg, dose reduced to 15 mg, daily), or warfarin. Bleeds were adjudicated using International Society on Thrombosis and Haemostasis criteria.

Among 3127 episodes of suspected infective endocarditis, the 2023 Duke-International Society for Cardiovascular Infectious Diseases clinical criteria showed an accuracy of 90% for infective endocarditis diagnosis. A new heart murmur was present in 690 (22%) episodes. Excluding imaging and surgical findings decreased the accuracy to 73%, while using the physical examination criterion slightly improved the accuracy to 78%.

Background: Disadvantaged socioeconomic status correlates with adverse outcomes for patients with congenital heart disease. We examined individual and neighborhood characteristics associated with adverse short-term surgical outcomes and investigated potential drivers of disparities.
Methods: Single-center retrospective analysis collected clinical and demographic information on cardiovascular surgery patients over a 15-year period (2007-2022) from the District of Columbia metropolitan area.
