Molecular docking to investigate HLA-associated idiosyncratic drug reactions.

Drug Metab Rev

January 2025

Department of Physiology and Pharmacology, Karolinska Institutet, Stockholm, Sweden.

Idiosyncratic drug reactions (IDRs) pose severe threats to patient health. Unlike conventionally dose-dependent side effects, they are unpredictable and more frequently manifest as life-threatening conditions, such as severe cutaneous adverse reactions (SCARs) and drug-induced liver injury (DILI). Some HLA alleles, such as , , and , are known risk factors for adverse reactions induced by multiple drugs.

Aim: To extend the findings of a previous clinical trial suggesting combined abacavir (ABC), lamivudine (3TC), and zidovudine (AZT) reduces type I interferon (IFN) signalling in Aicardi-Goutières syndrome (AGS).

Method: This was an open label, non-placebo-controlled phase II clinical trial (NCT04731103) in patients less than 16 years with any of five AGS genotypes. The effect of ABC or 3TC individually, or of combined ABC + 3TC + AZT, on IFN-stimulated gene (ISG) expression (primary outcome) and IFN-alpha protein (secondary outcome) in blood was assessed.

Evaluation of cephalosporin allergy: Survey of drug allergy experts.

J Allergy Clin Immunol Glob

February 2025

Division of Allergy, Pulmonary, and Critical Care Medicine, Vanderbilt University Medical Center, Nashville, Tenn.

Background: Since the publication of the 2022 Drug Allergy Practice Parameters (DAPP) of the American Academy of Allergy, Asthma & Immunology (AAAAI) and American College of Allergy, Asthma & Immunology (ACAAI), it is unclear the extent to which the simplified and risk-stratified evaluation of cephalosporin allergy has been incorporated into allergy practice.

Objective: We aimed to assess current cephalosporin allergy testing practices using real case examples.

Methods: An 18-question REDCap survey was sent to the 136 members of the Adverse Reactions to Drugs, Biologics and Latex (ARDBL) Committee of the AAAAI between February and April 2023.

Article Synopsis
  • The study aimed to determine if switching from dolutegravir/abacavir/lamivudine (DTG/ABC/3TC) to bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) reduces neuropsychiatric symptoms in HIV patients.
  • Conducted as a randomized trial, it involved 41 participants who had been stable on DTG/ABC/3TC and compared those who switched to BIC/FTC/TAF versus those who continued their current therapy.
  • Results indicated that switching improved sleep disorders among participants but did not significantly impact other neuropsychiatric symptoms, suggesting better tolerability with BIC-based therapy despite the small sample

COVID-19 and severe cutaneous allergic reactions to sulfonamides.

Allergy Asthma Proc

November 2024

From the Section of Allergy, Asthma and Immunology, Department of Medicine, Pennsylvania State College of Medicine, Hershey, Pennsylvania and.

Sulfonamides are associated with severe cutaneous adverse reactions (SCARs). Coronavirus disease 2019 (COVID-19) triggers an immune response, which may increase the likelihood of developing a hypersensitivity reaction. We sought to explore the impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection on the probability of developing SCARs and/or erythema multiforme (EM) reactions to sulfonamides.
