A case of a patient presenting a giant size mass compressing the SVC and IVC is presented. Chest X-ray, substernal echo-scanning, magnetic resonance tomography, etc. led to the hypothesis of a benign cardiac tumor or cardiac echinococcosis. However, surgical examination revealed a giant aneurysm of the coronary artery which was successfully treated.

Kawasaki disease, traditionally classified as medium vessel vasculitis, is known for the preferential involvement of coronary arteries. We report a case of a 14-year-old girl with complex congenital heart disease where computed tomography (CT) angiography demonstrated a giant aneurysm of the right inferior pulmonary vein, in the absence of any downstream obstruction. The case highlights the developmental aspects of this rare anomaly in addition to the role of CT angiography in anatomical depiction of structures which are difficult to visualize on transthoracic echocardiography.
