Lung cancer is commonly diagnosed at advanced stages, often presenting with metastases. Although bone metastases are common in lung cancer patients, acrometastases - metastatic lesions in the bones of the hand - are exceedingly rare. Herein, we report the case of a 71-year-old male with previously undiagnosed lung adenocarcinoma, which first manifested as a painful swelling in the right hand.

(Tricho-rhino-phalangeal syndrome 1) is a GATA transcriptional activator gene encoding for a protein used as a sensitive immunohistochemical marker of breast carcinomas. In dermatopathology, TRPS1 is used as a marker of mammary and extramammary Paget's disease and is also expressed by a variety of primary cutaneous tumors, mostly of adnexal origin. So far, very limited data exist on the expression of TRPS1 in metastatic skin tumors.

Introduction And Importance: Hand metastases are notably rare, comprising around 0.1% of all metastatic diseases, mainly originating from lung cancer, which is responsible for 30-40% of such cases. This report highlights a rare occurrence of distal phalangeal metastasis in a patient with Lynch syndrome, underscoring the diagnostic challenges associated with hand metastases.
