Current Practice and Outcomes of Patients Undergoing Surgical Resection for Renal Cell Metastases to the Pancreas in Northern Ireland.

Ulster Med J

December 2024

Department of Hepatobiliary and Pancreatic Surgery, Belfast Health and Social Care Trust, Belfast, UK.

Article Synopsis
  • * A study reviewed 15 patients who had pancreatic surgery for metastatic RCC over 18 years, finding that most had isolated pancreatic metastases and a long disease-free interval from their initial kidney tumor.
  • * Five-year survival rates post-surgery were 32.7% for disease-free survival and 63.3% for overall survival, with specific factors like synchronous cancer leading to shorter survival outcomes, while a longer gap before metastasis was associated with better overall survival trends.

Context: The advantage of intraoperative gamma probe detection with (111)In-DPTA-octreotide radiotracer has previously been demonstrated in functional neuroendocrine tumors. We report the only known case of a recurrent nonfunctional pancreatic neuroendocrine neoplasm localized intraoperatively using this radiotracer and a hand held gamma probe.

Case Report: A 51-year-old woman was found to have a recurrence 23 months after laparoscopic distal pancreatectomy, splenectomy and wedge resection of a liver metastasis for a non-functional neuroendocirne neoplasm of the pancreas.

Introduction: Pancreatic involvement in metastatic melanoma is not an uncommon feature in disseminated disease, but isolated metastasis to the pancreas is a rare entity. Management of the pancreatic metastatic melanoma is controversial and the role of surgery is poorly defined. Most of the resectable pancreatic metastases were treated by standard pancreatectomies, and the role of limited resections (as central pancreatectomy) was not yet established.
