Safe motherhood in Pakistan.

The maternal mortality ratio in Pakistan remains high at between 350 and 500 per 100,000 live births, while the neonatal mortality ratio is 50 per 1000 live births. This paper examines the trends in maternal and neonatal mortality in Pakistan and looks at why slow progress has been made in attempts to achieve safe motherhood. Despite numerous initiatives, it is uncertain whether Pakistan will achieve Millennium Development Goals 4 and 5 by 2015.

Puerperal inversion of uterus is a catastrophic and a rare complication, which occurs during the third stage of labour. It can lead to severe haemorrhage and shock which results in maternal morbidity and mortality. When diagnosed correctly, early cases can be managed by manual reposition, but neglected and late cases need surgery.

Background: Community acceptance of Traditional birth attendants (TBAs) as professional birth attendant was reported as one of the bottlenecks that has been hindering facility childbirth care service use. Hence, the World Health Organization (WHO) recommended that all childbirths needed to be attended by professional skilled attendants who at least possessed midwifery skill through the safe motherhood initiative.
