Considering that the world's population average age is increasing and the proportions of those over the age of 80, the fastest growing population worldwide, new approaches to health care and treatment will be necessary. Due to the development of new drugs and by means of transluminal coronary angioplasty or coronary artery bypass graft, many elderly patients had their lives prolonged and their quality of life improved. We carried out a bibliography review to search for evidence to support the best treatment choice according to the clinical manifestation of the patient.

Mixed Transcortical Aphasia (MTA) is an infrequent aphasic syndrome, characterized by poor comprehension and production in oral language abilities and poor performance in written language abilities. However, individuals with MTA typically retain the ability to repeat. Our patient, a woman who suffered from a left hemisphere ischemic stroke involving perisylvian areas, presented with repetition preserved for words, non-words, sentences and numbers, together with marginally preserved reading abilities.

White matter damage and subsequent demyelination significantly contribute to long-term functional impairment after ischaemic stroke. Identifying novel pharmacological targets to restore myelin integrity by promoting the maturation of oligodendrocyte precursor cells (OPCs) into new myelinating oligodendrocytes may open new perspectives for ischaemic stroke treatment. In this respect, previous studies highlighted the role of the G protein-coupled membrane receptor 17 (GPR17) as a key regulator of OPC differentiation in experimental models of brain injury, including ischaemic stroke.

To facilitate the implementation of controlled donation after circulatory death (cDCD) programs even in hospitals not equipped with a local Extracorporeal Membrane Oxygenation (ECMO) team (Spokes), some countries and Italian Regions have launched a local cDCD network with a ECMO mobile team who move from Hub hospitals to Spokes for normothermic regional perfusion (NRP) implantation in the setting of a cDCD pathway. While ECMO teams have been clearly defined by the Extracorporeal Life Support Organization, regarding composition, responsibilities and training programs, no clear, widely accepted indications are to date available for NRP teams. Although existing NRP mobile networks were developed due to the urgent need to increase the number of cDCDs, there is now the necessity for transplantation medicine to identify the peculiarities and responsibility of a NRP team for all those centers launching a cDCD pathway.

Article Synopsis
  • The study focuses on ulnar-basilic fistulas (UBFs) for hemodialysis, which are less common than radiocephalic fistulas due to specific anatomical challenges.
  • A retrospective review analyzed 11 UBFs created between 2008 and 2023, finding that while only 3.3% of functional distal AVFs were UBFs, many demonstrated immediate success.
  • The results indicated that UBFs are safe and durable options for hemodialysis despite technical difficulties, particularly for patients with multiple health issues.

Advances in medical and surgical interventions have resulted in a steady increase in the number of patients with congenital heart disease (CHD) reaching adult age. Unfortunately, this ever-growing population faces an added challenge: an increased risk of acquiring coronary artery disease. This review provides insight into the complex interactions between coronary artery disease and CHD in adults.
