Early portal vein thrombosis (PVT), after orthotopic liver transplantation (OLT), is relatively uncommon and is a serious complication that can compromise graft and patient survival. The factors predisposing to PVT after OLT include rejection, technical problems during the surgery, use of vein grafts and conduits, preoperative PVT, previous portasystemic shunts and splenectomy, hypercoagulable state, and significant development of gastroesophageal collaterals. Among them, few cases of PVT caused by coronary vein (CV) steal after OLT have been reported. Herein, we present a case with early PVT caused by CV steal after OLT, which is treated by a percutaneous transportal approach, with a review of the relevant literature.

Small-for-size syndrome is a clinical syndrome of early allograft dysfunction usually following living donor liver transplantation due to a mismatch between recipient metabolic and functional requirements and the graft's functional capacity. While graft size relative to the recipient size is the most commonly used parameter to predict risk, small-for-size syndrome is multifactorial and its development depends on a number of inter-dependant factors only some of which are modifiable. Intra-operative monitoring of portal haemodynamics and portal flow modulation is widely recommended though there is wide variation in clinical practice.

Background Kasai procedure (KPE) is a palliative intervention in infants with biliary atresia (BA) aiming to restore biliary drainage. While the measure of success in BA is the post-Kasai native liver survival, BA remains the most frequent indication for liver transplantation in children. While a considerable amount of children fail to clear their jaundice following KPE, resulting in early liver failure and transplantation, some children become jaundice-free after "successful" KPE.

Recently, arthroscopic anterior talofibular ligament (ATFL) repair has become popular, and favorable outcomes have been reported. In general, ATFL injuries are often caused by fibular attachment, and there are no reports of arthroscopic ligament repair of talar attachment injuries. We present a surgical technique for arthroscopic ligament repair via the anterolateral portal, accessory anterolateral portal, and far accessory anterolateral portal for ATFL injuries on the talar side.
