[Gynecological endoscopic surgery. Surgical approach by laparoscopy].

We report the first 32 patients in whom surgical procedures were performed through laparoscopy. We practiced adhrenciolysis, biopsy and fenestration of cystic ovarian lesions, section of uterosacral ligaments as a treatment for pelvic pain, ovaric biopsies in cases of premature ovarian failure, fulguration of endometriotic implants, myomectomy, conservative management of an hemorrhagic corpus luteum, linear salpingostomy and total salpingectomy in ectopic pregnancy. The average operating time was 45 minutes, and the postoperative stay 28 hours. The only complication was active tubal bleeding in an ectopic pregnancy initially approached through linear salpingostomy, that required laparotomy. We describe the utilized technique, discussing the most frequent indications, and also, the basic elements necessary for performing laparoscopy surgery.

The aim of this study was to improve clinical decision-making for the identification of persistent ectopic pregnancy after linear salpingostomy. The study identified 854 laparoscopic salpingostomies performed between 2011 and 2016; 794 had a human chorionic gonadotrophin (HCG) <10 mIU/ml documented in the electronic medical record within 1 month after surgery ('successes'). Sixty (7%) received either methotrexate or repeat surgery for persistent ectopic pregnancy ('failures').

Aim: The objective of this study was to review our experience with ectopic pregnancy management and to evaluate major predictive factors for failure of conservative linear salpingostomy.

Methods: Seven hundred and ninety-seven cases that were treated for ectopic pregnancy in our clinic between October 2005 and September 2011 were analyzed retrospectively for incidence and treatment options. We defined failure as rising or plateauing postoperative serum β-hCG levels that required additional medical or surgical treatment after initial removal of the ectopic pregnancy by laparoscopy or by laparotomy.
