Results are reported of chemical (derivatographic and polarization microscopic) analysis of 2630 calculi removed by operation and 8607 spontaneously eliminated calculi. It was found that 82.7 per cent of the calculi removed by operation and 19.4 per cent of those eliminated spontaneously were mixed--composed of two or more components. The most common "pure" concrements were composed from oxalates and uric acid. The difference in the chemical types of the renal calculi of the two compared groups (removed by operation and spontaneously) is shown and followed up. Of interest is the different incidence of the two calcium oxalate hydrates in the two compared groups of renal calculi. Vevelite was more common in calculi removed by operation, while vedeline was the more common form in spontaneously eliminated calculi. Precision of the chemical form of the calculosis is recommended, with a view to including the genesis of stone formation for effective prophylaxis, treatment and metaphylaxis of the disease.

The surgical management of cancer patients wishing fertility preservation is multidisciplinary, involving surgeon, anesthetist, hematologist, and nursing and laboratory staff. Many oncology patients have a multitude of medical or surgical conditions that require careful planning of all therapy including surgical removal of reproductive material, either oocytes or ovarian tissue. The significant risks related to either transvaginal or abdominal surgery should be discussed and documented and the final decision to proceed must be balanced against the risks, including death.

Infectious myocarditis (IM) and infective endocarditis (IE), sometimes associated with infection of the surrounding mediastinal tissue or embolic complications caused by residual implantable cardioverter defibrillator (ICD) lead material embedded in the ventricle, present a significant challenge for cardiac surgeons due to the difficulty of precisely locating the old intracardiac pacing lead remnants because of the heart's continuous movement. We present the case of successful two-stage elective sternotomy extraction of two residual defibrillator leads, one trapped in the left innominate vein, easily removed after veinotomy without cardiopulmonary bypass (CPB), and the other embedded intramyocardially in the inferior wall of the right ventricle, successfully removed under CPB after fluoroscopic guidance. The patient was discharged four weeks post-operation without complications.
