Comparison of Post-auricular Soft Tissue and Post-auricular Soft Tissue With Autologous Bone Pate in Obliteration of the Canal Wall Down Mastoidectomy Cavity.

Cureus

December 2024

Department of Otolaryngology, Head and Neck Surgery, Sri Devaraj Urs Academy of Higher Education and Research, Kolar, IND.

Background The surgical management of chronic otitis media (COM) with squamous disease is canal wall down mastoidectomy (CWDM). Canal wall down procedures require the obliteration of the newly formed cavity to mitigate complications. Soft tissue flaps, including Rambo flap, Hong Kong flap, Palva flap, and inferior-based fascio-periosteal flap, as well as autologous bone pâté, have been the most successful and commonly used materials for obliteration over the past two decades.

Surgical Management of Pulsatile Tinnitus due to Sigmoid Sinus Diverticulum and Dehiscence - A Technical Note.

Indian J Otolaryngol Head Neck Surg

Pulsatile tinnitus (PT) is the perception of an auditory sensation without an external source and in synchrony with the heartbeat. This case report describes a 26-year-old female patient who presented with pulsatile tinnitus caused by sigmoid sinus diverticula and dehiscence, which was successfully treated with cortical mastoidectomy with diverticula closure using pedicled temporalis fascia and resurfacing of the dehiscence with autogenous bone pate along with bone cement.

Objective: To evaluate the efficacy of reduction meatoplasty, a novel technique aiming to improve the usage of molded hearing aids and ear plugs in patients undergoing mastoid obliteration of a previous canal wall-down (CWD) mastoidectomy, complicated with a very large meatus.

Patients: Twenty-six patients from a tertiary care neurotology clinic with a very large meatus from a previous CWD mastoidectomy, who were unable to use molded hearing aids or ear plugs for water protection.

Intervention: Reduction meatoplasty after mastoid obliteration (cartilage strips and bone pate) of the CWD mastoidectomy.

Background: Cholesteatoma surgery involves canal wall down (CWD) and canal wall up (CWU) mastoidectomy. CWU is associated with higher cholesteatoma recurrence, often linked to attic retraction pockets. Attic reconstruction with cartilage or bone pate lacks comparative evidence.
