Chronic ventricular lead dislodgement is an infrequent complication of pacemaker implantation. Occasionally, the dislodged lead may sense and capture a chamber in which the lead was not originally positioned. Intracardiac real time electrograms and channel markers are useful tools for the diagnosis of pacemaker malfunction. We present the case of a patient with a ventricular lead dislodgement into the atrium. The ventricular lead was able to sense and capture the atrium. Initial diagnosis was performed based on the deductive analysis of intracardiac real time electrograms and channel markers and confirmed by chest X-ray.

Functional mitral regurgitation is characterized by normal structures of the mitral valve and chordae tendinea, but the regurgitation occurs due to geometric changes in the left atrium and left ventricle. This condition can contribute to heart failure progression and lead to a poor prognosis. Functional mitral regurgitation is found in approximately one-third of patients with heart failure with a decreased ejection fraction.

In the context of dynamic image-based computational fluid dynamics (DIB-CFD) modeling of cardiac system, the role of sub-valvular apparatus (chordae tendineae and papillary muscles) and the effects of different mitral valve (MV) opening/closure dynamics, have not been systemically determined. To provide a partial filling of this gap, in this study we performed DIB-CFD numerical experiments in the left ventricle, left atrium and aortic root, with the aim of highlighting the influence on the numerical results of two specific modeling scenarios: (i) the presence of the sub-valvular apparatus, consisting of chordae tendineae and papillary muscles; (ii) different MV dynamics models accounting for different use of leaflet reconstruction from imaging. This is performed for one healthy subject and one patient with mitral valve regurgitation.

Introduction: This study evaluates various formulae used to correct the QT interval in patients with wide QRS complexes to calculate corrected QT (QTc) following Cardiac Resynchronisation Therapy (CRT).
Methods: We included patients with severe heart failure and left bundle branch block, presenting with a QRS duration of at least 120 milliseconds, who underwent successful CRT implantation. Patients were excluded if they had non-lateral left ventricular lead placement, metabolic disorders, atrial fibrillation, atrial tachycardia, or high-degree atrioventricular block prior to implantation.

Brugada syndrome (BrS) is an inherited arrhythmogenic disorder characterized by distinct electrocardiographic patterns and an increased risk of sudden cardiac death due to ventricular arrhythmias. Effective management of BrS is essential, particularly for high-risk patients with recurrent arrhythmias. While implantable cardioverter-defibrillator (ICD) is effective in terminating life-threatening arrhythmias, it does not prevent arrhythmia onset and can lead to complications such as inappropriate shocks.
