The role of adiposity as a determinant of an inflammatory milieu.

With the growing prevalence of obesity, scientific interest in the biology of adipose tissue has been extended to the secretory products of adipocytes, since they have been shown increasingly to affect several aspects of the pathogenesis of obesity-related diseases. Until relatively recently, the role of adipose tissue itself in the development of obesity and its consequences was considered to be a passive one. It is now clear that, in addition to storing energy in the form of triglycerides, adipocytes also secrete a large variety of proteins, including cytokines, chemokines and hormone-like factors. This production of proatherogenic chemokines by adipose tissue is of particular interest, since their local secretion, for example by perivascular adipose depots, may provide a novel mechanistic link between obesity and associated vascular complications.

Visceral adipose tissue, a type of abdominal adipose tissue, is highly involved in lipolysis. Because increased visceral adiposity is strongly associated with the metabolic complications related with obesity, such as type 2 diabetes and cardiovascular disease, there is a need for precise, targeted, personalized and site-specific measures clinically. Existing studies showed that ectopic fat accumulation may be characterized differently among different populations due to complex genetic architecture and non-genetic or epigenetic components, ie, Asians have more and Africans have less visceral fat vs Europeans.

Background: The association between skeletal muscle and adipose tissue (body composition) and early response using positron emission tomography (PET) in pediatric Hodgkin lymphoma (HL) remains unstudied.

Methods: Patients enrolled on Children's Oncology Group studies AHOD0031 (intermediate-risk HL) and AHOD0831 (high-risk HL) with digital abdominal computed tomography (CT) scans at diagnosis and PET scans after 2 cycles (PET2) were included. Two consecutive slices at the third lumbar vertebra were identified and skeletal muscle index (SMI, in cm2/m2) and total adipose tissue index (TATI, in cm2/m2) were calculated using sliceOmatic (Magog, Canada) and height at diagnosis.
