Background And Methods: Eight limited sampling strategies (LSSs) for estimating mycophenolic acid area under the concentration-time curve (4 developed from lung transplant recipients at our center, 4 developed for heart transplant recipients from other research groups) were evaluated in 27 heart or heart-kidney transplant patients.
Results: The LSSs from our lung transplant patients performed well when applied to the heart transplant population, with percent bias and percent precision within the acceptable limit of +/-15%.
Conclusions: The LSSs developed at our center are robust enough to be applied to both lung and heart transplant populations. Application of LSSs from other research groups yielded less optimal results, reinforcing the need to re-establish or re-validate LSSs for each specific center.

Vascularized composite allotransplantation (VCA) represents a clinical challenge for transplant therapy, as it involves different tissues with unique immunogenicity. Even when receiving immunosuppressive therapy, they are more vulnerable to severe hypoxia, microvascular damage, and ultimately the rejection or chronic graft dysfunction after transplantation. This study aimed to develop a surgical protocol for VCA of the ear in a porcine biomodel in the absence of immunosuppression, maintaining the in vitro co-culture of the allograft and assessing their relationship with allograft survival.

Background: Predicted heart mass ratio (PHMr) has become the standard donor-recipient size matching method in heart transplantation. While utilization of small PHMr hearts is associated with increased one-year mortality, the underlying mechanisms and time horizon of mortality remain uncertain.
Methods: A single institution analysis of isolated heart transplant recipients (01/2019-7/2022) was performed (N=334).

Background: While metagenomic next-generation sequencing (mNGS) has been acknowledged as a valuable diagnostic tool for infections, its clinical validity and impact on patient management when using fresh tissue samples remains uncertain.
Methods: We conducted a retrospective cross-sectional study involving patients who underwent tissue mNGS at a tertiary hospital in China from February 2021 to February 2024, aiming to assess its ability to detect plausible pathogens and its clinical validity and impact.
Findings: A total of 520 mNGS results from 508 patients were analysed, detecting plausible pathogens in 302 (58.

Voclosporin is a potent immunosuppressive agent currently approved for treating active lupus nephritis. Based on its potential antiviral activity, it has also been investigated as immunosuppressive agent in an investigator-initiated study in SARS-CoV2 positive kidney transplant recipients. As with many immunosuppressive agents, optimizing dosing regimens to achieve therapeutic efficacy while minimizing toxicity remains a critical challenge in clinical practice.

Introduction: Given the risks of cardiovascular disease among pediatric kidney transplant recipients, we evaluated whether there was an association between rapid weight gain (RWG) following kidney transplantation and the development of obesity and hypertension among children enrolled in the North American Pediatric Renal Trials and Collaborative Studies (NAPRTCS) registry.
Methods: This retrospective analysis of the NAPRTCS transplant cohort assessed for RWG in the first year post-transplant and evaluated for obesity and hypertension in children with and without RWG up to 5 years post-transplant. We evaluated three separate eras (1986-1999, 2000-2009, and 2010-2021).
