[Transesophageal echocardiography: a new view of the heart].

Transoesophageal echocardiography is a simple technique which is now the object of a growing and well-deserved interest, due to the wealth of information it provides. This examination does not supersede the conventional transthoracic echocardiography, but it completes it by supplying more accurate data, especially in difficult clinical cases, such as mitral valve pathology, cardiac valve prostheses, intracardiac thrombosis, aortic dissection and septal abnormalities.

Background: Historically, percutaneous transcatheter left atrial appendage closure (LAAC) has been performed under general anesthesia (GA) with transesophageal echocardiographic images obtained by a noninvasive cardiologist and usually requires an overnight hospital stay. Transcatheter edge-to-edge repair (TEER) is an effective intervention for high-risk patients with severe symptomatic mitral regurgitation (MR), but its acute impact on left ventricular (LV) function has not been well studied using advanced echocardiographic techniques. This study investigated the immediate effects of TEER on LV volumes and functions, as well as their influence on mid-term outcomes, using high-resolution 3D transesophageal echocardiography. In 80 patients undergoing TEER for severe MR (mean age 79±8 years, 49% with primary MR), LV end-diastolic volume (LVEDV) and stroke volume significantly decreased (161±61 to 147±54 ml and 69±18 to 50±15 ml, respectively), while end-systolic volume increased (92±60 to 97±45 ml; p<0.

Performing automatic and standardized 4D TEE segmentation and mitral valve analysis is challenging due to the limitations of echocardiography and the scarcity of manually annotated 4D images. This work proposes a semi-supervised training strategy using pseudo labelling for MV segmentation in 4D TEE; it employs a Teacher-Student framework to ensure reliable pseudo-label generation. 120 4D TEE recordings from 60 candidates for MV repair are used.
