Resting tremor (RT) is a Parkinson's disease (PD) symptom with an unclear relationship to the dopaminergic system. We analysed data from 432 subjects from the Parkinson's Progression Markers Initiative, 57 additional PD patients and controls and 86 subjects referred for dopamine transporter single-photon emission computed tomography (DaT-SPECT). Caudate binding ratio (CBR), but not putamen binding ratio, was higher in RT patients.

Background: Cognitive reserve (CR) is an expression of brain resilience in response to damage. Education, occupational experience and leisure activities are thought to increase CR and have beneficial effects on global cognition and cognitive decline in Parkinson's disease (PD). We aimed to disclose brain metabolic and dopaminergic correlates of CR in de-novo PD patients.

Is speech function lateralised in the basal ganglia? Evidence from de novo Parkinson's disease.

J Neurol Neurosurg Psychiatry

September 2024

Movement Disorders Center, Department of Neurology, University Hospital of Bern, Bern, Switzerland.

Background: Research on the possible influence of lateralised basal ganglia dysfunction on speech in Parkinson's disease is scarce. This study aimed to compare speech in de-novo, drug-naive patients with Parkinson's disease (PD) with asymmetric nigral dopaminergic dysfunction, predominantly in either the right or left hemisphere.

Methods: Acoustic analyses of reading passages were performed.

Article Synopsis
  • The akinetic crisis is a severe, potentially life-threatening condition in Parkinson's disease characterized by extreme rigidity and akinesia that does not respond to levodopa, often accompanied by high fever and altered consciousness.
  • Although akinetic crises are rare, milder forms of motor deterioration in Parkinson's patients frequently lead to hospitalizations, highlighting the broader spectrum of akinetic symptoms.
  • The review explores clinical symptoms, risk factors, diagnostic methods like DAT-SPECT for assessing levodopa-resistance, and discusses the role of inflammatory cytokines in progression, along with guidelines for managing these conditions.

Background: Pseudo-orthostatic tremor is a hyperkinetic movement disorder usually associated with other neurological comorbidities, mainly Parkinson's disease.

Case Report: A 65-year-old male presented with unsteadiness and leg tremor while standing. Electrophysiological evaluation confirmed the presence of pseudo-orthostatic tremor.
