Resusc Plus

January 2025

Department of Emergency Medicine, University Hospital Gasthuisberg, Leuven, Belgium.

A 36-year-old woman at 23 weeks and 3 days of gestation experienced a witnessed cardiopulmonary collapse. Bystander cardiopulmonary resuscitation (CPR) was initiated immediately. After advanced life support, she was transferred under mechanical CPR to a hospital for extracorporeal membrane oxygenation (ECMO).

Resuscitation

December 2024

University Hospitals Birmingham NHS Foundation Trust, Birmingham Heartlands Hospital, B9 5SS, UK; Warwick Clinical Trials Unit, University of Warwick, Coventry CV4 7AL, UK.

Objective: To examine maternal and neonatal outcomes following Resuscitative Hysterotomy for out of hospital cardiac arrest (OHCA) and to compare with timing from cardiac arrest to delivery.

Methods: The review was registered with PROSPERO (CRD42023445064). Studies included pregnant women with out of hospital cardiac arrest and resuscitative hysterotomy performed (in any setting) during cardiac arrest.

Article Synopsis
  • Simulation-based learning is crucial for emergency physicians to effectively handle rare obstetric emergencies, particularly maternal cardiac arrest, which involves a key procedure called resuscitative hysterotomy (RH).
  • A low-cost RH task trainer was developed to improve the skills and confidence of emergency medicine residents, with a 20-minute scenario involving a pregnant trauma patient.
  • Feedback from 30 participating residents indicated that the simulation was effective, enhancing their clinical skills and confidence, while future plans aim to improve the trainer's realism and accessibility.

Article Synopsis
  • Trauma during pregnancy poses risks for both the mother and fetus, necessitating interventions like resuscitative hysterotomy (RH) to improve maternal outcomes alongside fetal survival.
  • Recent studies indicate that RH can quickly restore maternal vital signs, making it crucial for patients with unstable conditions before cardiovascular collapse.
  • A case review of 4 pregnant trauma patients showed a 50% maternal survival and provided insights into the effectiveness of RH in emergency situations, highlighting a shift towards prioritizing maternal health.
