[Bilateral non-union scaphoid or bipartite carpal scaphoid bone arthritis?].

The authors report a case of bilateral division of the carpal scaphoid in a 36-year-old man. In the right wrist, all the clinical and radiological signs suggested a bipartite scaphoid, but in the left wrist the presence of pain and pan-radial osteoarthritis rather suggested a pseudarthrosis of the scaphoid. This raises the possibility of osteoarthritis occurring overtime in a bipartite carpal scaphoid.

Introduction: Rhizarthrosis, or osteoarthritis of the trapeziometacarpal joint, predominantly affects women over 50, with up to 30% experiencing some degree of arthritis in this joint. TMC ball-in-socket arthroplasty, with a cup placed in the distal scaphoid, offers a promising alternative to traditional arthrodesis or resection-suspension arthroplasty.

Surgeons use anatomical landmarks like the scaphoid tubercle, pisiform, trapezial tubercle and hook of hamate, along with Kaplan cardinal line (KCL) to avoid injury to the recurrent motor branch (RMB) of the median nerve during carpal tunnel release. The presence of transverse muscle fibres (TMF) overlying the transverse carpal ligament (TCL) may suggest proximity of the RMB, but their anatomical relationship is unclear. In this study, we evaluated the accuracy of anatomical landmarks to the RMB, TMF origin and insertion, and examined the relationship between TMF presence and RMB running patterns.
