Displacement of retained subfoveal perfluorocarbon liquid after vitreoretinal surgery.

Objective: To report a novel surgical procedure to displace retained subfoveal perfluorocarbon liquid (PFCL).

Methods: Retrospective cases series. Three patients had retained subfoveal droplets after PFCL was used in vitrectomy repair of retinal detachment. In each case, submacular PFCL was displaced to the subretinal space in the inferior periphery. A retinal detachment at the posterior pole and the inferior periphery was created by injecting balanced salt solution through a retinal puncture near the inferotemporal vessels. Fluid-air exchange was performed, followed by short-term postoperative upright head positioning.

Results: In all cases, PFCL droplets were successfully displaced toward the inferior periphery, with good visual recovery.

Conclusion: This procedure seems to be safe and is an alternative to direct aspiration of PFCL through a juxtafoveal retinotomy, which risks damage to the foveal region.

Geographic atrophy (GA) is an advanced form of age-related macular degeneration (AMD), characterized by atrophic lesions that first start in the outer retina and progressively expand to cover the macula and the fovea, the center of the macula, leading to irreversible loss of vision over time. GA is distinct from wet or neovascular AMD (nAMD), the other form of advanced AMD. Neovascular AMD is characterized by new invading leaky blood vessels in the macula that can lead to acute vision loss.

Purpose: To report unprompted closure of spontaneous macular hole secondary to inadvertent subfoveal perfluorocarbon liquid (PFCL) after vitrectomy surgery. . We present a case of a retained large single subfoveal PFCL droplet following vitrectomy and silicone oil injection for subtotal rhegmatogenous retinal detachment that showed spontaneous release 3 weeks postoperatively, with subsequent development of full thickness macular hole (FTMH) which completely closed later on after silicone oil removal without internal limiting membrane peeling with marvelous anatomic and visual improvement after spontaneous closure of MH.
