As the threshold of viability decreases, clinicians who care for preterm neonates increasingly are faced with making decisions regarding feeding in a population for which there are no clear guidelines and minimal scientific data to inform optimal nutritional strategies. It is becoming more apparent that the amount of nutrition, as well as the timing of nutrient delivery, may affect long-term growth and metabolic outcomes. Both overnutrition and undernutrition at critical growth periods may have adverse effects. The risks and benefits of nutrient delivery designed to achieve "catch-up" growth need to be further identified. This article reviews both established concepts and more recently identified issues that need to be considered by the neonatologist when making feeding decisions in the very preterm neonate.

Starch spherulite is a unique form of resistant starch characterized by a spherical structure with crystalline lamellae that are radially oriented and may find applications in delivery of nutrients and bioactives to the lower gastrointestinal tract. Formation of starch spherulites generally requires heating to a high temperature followed by quenching and long crystallization time. The objectives of this study were to gain a deeper understanding of the factors influencing spherulite formation from pea starch (PS) and high-amylose maize starch (HAMS) and investigate if spherulites could be formed by a slow cooling rate and determine the crystalline structure and morphology of the spherulites formed.

Human milk oligosaccharides (HMOs), the third most abundant solid component in human milk, vary significantly among women due to factors such as secretor status, race, geography, season, maternal nutrition and weight, gestational age, and delivery method. In recent studies, HMOs have been shown to have a variety of functional roles in the development of infants. Because HMOs are not digested by infants, they act as metabolic substrates for certain bacteria, helping to establish the infant's gut microbiota.

This study aimed to adapt evidence-based diabetes self-management education and support (DSMES) into a faith-based (FB) context for Hispanic communities and compare its effectiveness to a faith-placed (FP) approach using the church as a venue for DSMES delivery.
