We present the case of a 62-year-old woman who presented with recoarctation and then experienced rupture of the aorta and severe clinical deterioration after a stent was deployed. She was treated immediately by intrastent deployment of a stent-graft, which resolved the extremely serious situation.

Background: Previous studies have strongly suggested that stent-graft deployment and acute arch angulation increase aortic stiffness, the impact of surgical interposition grafting remains unclear. We investigated the impact of open surgery on aortic stiffness and compared this with stent-graft induced aortic stiffening, utilising an ex vivo model.
Methods: Porcine thoracic aortas were connected to a mock circulatory loop.

An 86-year-old male with multiple medical comorbidities was referred for an elective transcatheter aortic valve replacement (TAVR) for severe symptomatic aortic stenosis with an ejection fraction of 35%. A self-expanding valve was deployed successfully, but intraoperative transesophageal echocardiography (TEE) confirmed an acute type A aortic dissection (ATAAD), necessitating immediate intervention. Multiple intraoperative imaging modalities were utilized for deployment of a conformable GORE TAG (cTAG) stent graft for thoracic endovascular aortic repair (TEVAR).
