Purpose Of Review: The current system of kidney allocation in the United States has been undergoing review by the Organ Procurement and Transplantation Network. The Organ Procurement and Transplantation Network kidney committee is nearing the release of a draft proposal for sweeping changes in kidney allocation. The involvement of the renal community is critical to the successful development and implementation of these new policies.
Recent Findings: This review describes ethical issues related to allocation of scarce resources such as kidneys, the current US kidney allocation system and problems associated with it, as well as the development of a new model for kidney allocation. The new paradigm involves the application of 'life years from transplant'--an approach that compares predicted survival of a recipient after transplant versus remaining on dialysis, adjusted for multiple factors such as cause of renal failure, age and co-morbid conditions. Alternate systems as well as criticism of the current proposals are presented.
Summary: The use of life years from transplant would increase allocation of kidneys to patients more likely to benefit from transplantation, resulting in many thousands of additional life-years saved. Such a scheme would also tend to assign these kidneys to younger patients, which has generated controversy.

Background: Implementation of semaglutide weight loss therapy has been challenging due to drug supply and cost, underscoring a need to identify those who derive the greatest absolute benefit.
Objectives: Allocation of semaglutide was modeled according to coronary artery calcium (CAC) among individuals without diabetes or established atherosclerotic cardiovascular disease (CVD).
Methods: In this analysis, 3,129 participants in the MESA (Multi-Ethnic Study of Atherosclerosis) without diabetes or clinical CVD met body mass index criteria for semaglutide and underwent CAC scoring on noncontrast cardiac computed tomography.

Introduction: HLA matching is critical for successful kidney transplantation. This study aimed to investigate the impact of eplet mismatches and Predicted Indirectly Recognizable HLA Epitopes (PIRCHE-II) scores on the development of de novo donor-specific antibodies (dnDSA) and graft survival in a Tunisian cohort, characterized by a high prevalence of living donors and significant genetic diversity in HLA profiles.
Methods: This retrospective study included 112 adult kidney transplant recipients who underwent transplantation between 2012 and 2018.

Cardiology, a high-acuity medical specialty, has traditionally emphasised technical expertise, often overshadowing the critical role of non-technical skills (NTS). This imbalance stems from the historical focus on procedural competence and clinical knowledge in cardiology training and practice, leaving a significant gap in the development of crucial interpersonal and cognitive abilities. However, emerging evidence highlights the significant impact of NTS on patient outcomes, team dynamics, and overall healthcare efficiency.

Purpose: Preoperative C-reactive protein (CRP) is a valuable prognostic biomarker in nonmetastatic clear cell renal cell carcinoma (nmccRCC). Incorporation of CRP into prognostic models may improve the prediction of oncologic outcomes. Herein, we aimed to develop and validate prognostic nomograms and an integrated software incorporating preoperative CRP level in nmccRCC.

Purpose: Percutaneous nephrolithotomy (PCNL) as an established procedure for treatment of large kidney stones, can trigger life threatening complications. Postoperative hemorrhaging is one of the main complications of PCNL. This study investigates the effectiveness of balloon nephrostomy in reducing hemorrhage in the postoperative phase of PCNL.
