This article describes some of the salient anatomic issues, a classification system, surgical techniques, and a decision-making algorithm available for contouring the gluteal region in the patient who has sustained massive weight loss (MWL). The gluteal deformities encountered in patients who have lost a massive amount of weight are unprecedented in body-contouring surgery, and plastic surgeons need uncommon techniques when contouring and augmenting the gluteal region in these patients. A better understanding of the anatomy and a new surgical armamentarium can improve the cosmetic results of gluteal contouring, and, thereby, enhance patient satisfaction in this challenging population.

The gluteal region has become a rising area of interest in plastic surgery in recent years, as reflected in body contouring surgery trends. In this study, the authors explain what the arrow effect consists of and how to highlight it during a Brazilian butt lift or a gluteal liposculpture: liposuction of the sacral triangle, fat grafting of the upper gluteal quadrants, liposuction of the dimples of Venus and of the midline superficially to the spinal column, and fat grafting of the erector spinae muscles.

Background: Autologous fat grafting in the buttocks has gained great popularity However, one of the main risks is infiltration of fat into the systemic venous system, leading to potential complications such as macroscopic fatty embolism (MAFE), which can be fatal.
Objectives: This study evaluated the safety and efficacy of the LiRi (Linear Retrograde infiltration) for autologous fat grafting in the buttocks, and its evolution over a six-month follow-up period.
