Isolated pancreatic islets from thiamine deficient rats secrete less insulin. The secretion of insulin in response to glucose and tolbutamide is also decreased in these islets. Glucose and pyruvate oxidations to CO2, were decreased in the islets isolated from thiamine deficient rats. In the islets from control but not from thiamine deficient rats the oxidations of glucose and pyruvate to CO2 were increased by tolbutamide. The results suggest that in thiamine deficiency the insulin secretion is impaired due to the decreased glucose oxidation.

Wernicke's encephalopathy is a potentially lethal complication of thiamine deficiency which mainly occurs in chronic alcoholic patients. It may occur in other conditions like hyperemesis gravidarum too. Pregnancy may also be complicated with other neurological and cardiac complications.

Thiamine and pyridoxine are essential B vitamins that serve as enzymatic cofactors in energy metabolism, protein and nucleic acid biosynthesis, and neurotransmitter production. In humans, thiamine transporters SLC19A2 and SLC19A3 primarily regulate cellular uptake of both vitamins. Genetic mutations in these transporters, which cause thiamine and pyridoxine deficiency, have been implicated in severe neurometabolic diseases.

Accurate and selective monitoring of thiamine levels in multivitamin supplements is essential for preventing deficiencies and ensuring product quality. To achieve this, a Förster resonance energy transfer (FRET) system using carbon dots (CDs) as energy donors and citrate-stabilized silver nanoparticles (AgNPs) as energy acceptors was developed. The aqueous synthesis of AgNPs using microwave irradiation was optimized to obtain efficient plasmonic nanoparticles for FRET applications, targeting maximal absorbance intensity, stability, and wavelength alignment.

Introduction: Wernicke encephalopathy is a clinical diagnosis that requires a high degree of clinical suspicion to recognize. We report a case of a pregnant patient developing Wernicke encephalopathy in the setting of severe hyperemesis gravidarum.
Case Report: The patient was a 22-year-old female 13 weeks pregnant presenting to the emergency department (ED) with neurological deficits after several weeks of hyperemesis gravidarum requiring hospitalization.
