Purpose: To explore available literature on PNS mucoceles and its distortions of craniofacial-orbital anatomy with regard to orbital bony defects and ophthalmic manifestations, highlighting the PNS mucoceles that mostly result in these distortions.
Methods: A comprehensive literature search was conducted in June 2024 for available literature on the subject matter viz.; Google Scholar, PubMed and Medline, and Cochrane Library.
Hydrocephalus is the disruption of cerebral spinal fluid homeostasis, representing a common neurosurgical illness. Up to 10% have no identifiable cause, with fourth ventricle outflow obstruction (FVOO) being an extremely rare subtype. A 31-year-old male with a history of idiopathic hydrocephalus for over 10 years with the need for a ventriculoperitoneal shunt had shown progressively enlarged tetraventricular ventriculomegaly.

Introduction: Tolosa-Hunt syndrome (THS) is a disorder related to inflammation of cavernous sinus and superior orbital fissure that usually presents with ophthalmoplegia and oculomotor nerve palsies. The etiology of the syndrome is unknown and the diagnosis is set by exclusion of other clinical conditions that manifest in a similar way. Intracranial aneurysms, such as intracavernous ones, should be included in the differential diagnosis as they can compress cranial nerves leading to similar clinical presentation.
